Pre-Register for Your Visit
Save time by completing the pre-registration process online prior to any non-emergent appointment at Nacogdoches Medical Center. You can fill out your paperwork in the convenience of your home, at work or anywhere you have access to a computer. It’s a convenient way to prepare for your visit before you get to the hospital.
When you arrive, please have your:
• Driver’s license, state identification card or any other form of picture ID
• Insurance identification card
• Your doctor’s order (if any)
• A referral (if required by your insurance carrier)

To get started, simply click here. When you include a valid email address, we’ll send you a confirmation email within 72 hours that we have received your paperwork. We’ll also let you know if we need more information, but you may need to complete additional paperwork in person.
ER and Appointment Request
To check in to the ER, use our ER check-in tool. To find a physician, we offer an online physician look up, too.
